Check with your insurance carrier prior to your appointment to see if your plan requires an authorization to see the doctor. Failure to do so will result in the balance for the date of service in question becoming YOUR responsibility.

We take all major medical insurance (including Worker’s Compensation).

We DO NOT take new patients who have Medicare+Medicaid combined.
